docs(10-03): complete report-signoff plan and close Phase 10

- Mark Phase 10 complete in ROADMAP.md (3/3 plans, 2026-04-13)
- Create 10-03-report-signoff-SUMMARY.md with attestation-gap closure note
- Update STATE.md: plan 10-03 complete, Phase 10 closed, next is phase 8 or 11
- REQUIREMENTS.md RTVAL-01..05 ticks already landed in 10-02 completion commit (idempotent)
